We accept referrals from:

• Local authority and NHS commissioners
   
• Hospital discharge and flow teams
   
• Community mental health, learning disability, and autism teams
   
• Social workers and care coordinators
   
• Case managers and other professionals
   
• Individuals, families, and carers who wish to explore support options
   

We provide support for:

• Adults aged 18 and over
   
• People with mental health needs, learning disabilities, and autistic people
   
• Individuals living in their own homes or supported living settings in [insert local areas/regions]
   
• People who would benefit from home-based support, step-down from hospital, or intensive community support to prevent admission
   

1. Initial screening – We review the referral to check eligibility, risk, and whether we are the right service.
    
2. Assessment – We arrange a face-to-face or virtual assessment with the person (and their family or carers if they wish) to understand needs, strengths, and goals.
    
3. Support planning – We develop a person-centred care and support plan, including any personal care, daily living skills, and mental health support required.
    
4. Start of support – Once funding and arrangements are agreed, we work with the individual and partners to agree a start date and introduce the support team.
