Who can make a referral?
We accept referrals from:
- Local authority and NHS commissioners
- Hospital discharge and flow teams
- Community mental health, learning disability, and autism teams
- Social workers and care coordinators
- Case managers and other professionals
- Individuals, families, and carers who wish to explore support options
Who we can support
We provide support for:
- Adults aged 18 and over
- People with mental health needs, learning disabilities, and autistic people
- Individuals living in their own homes or supported living settings in [insert local areas/regions]
- People who would benefit from home-based support, step-down from hospital, or intensive community support to prevent admission
What happens after a referral?
- Initial screening – We review the referral to check eligibility, risk, and whether we are the right service.
- Assessment – We arrange a face-to-face or virtual assessment with the person (and their family or carers if they wish) to understand needs, strengths, and goals.
- Support planning – We develop a person-centred care and support plan, including any personal care, daily living skills, and mental health support required.
- Start of support – Once funding and arrangements are agreed, we work with the individual and partners to agree a start date and introduce the support team.
